MAKE A REFERRAL

This referral must be made through Morrison County Case Manager or Northern Pines Case Manager.

1. Please fill out the attached referral form and email it to aatchley@acardinalsjourney.com or fax it to (320)-238-7659.

2. Fill out referral paperwork. (You can download them from below and either email or fax them.)

3. You will be notified within 48 hours whether you will be sent opening paperwork or will placed on a waitlist. (If you do not hear back within that time period, please call or email.)

4. Fill out opening paperwork. (These will be sent from Jane.)

• Insurances Accepted: Medicaid, BCBS, United Healthcare, UMR, Medica, Aetna

• Private Pay
  • Individual Therapy: $125
  • Family/Marriage Therapy: $150

 Payments can be made by cash, check, or credit card.
